MEMBERSHIP APPLICATION

I wish to become a Member of AMERICANS FOR HEALTHCARE ALTERNATIVES (AFHA) in good faith for the purpose of enhancing my God-given rights. By becoming a member of AFHA, I agree to the following terms and conditions: 

AFHA is organized in part for providing wellness to the people. The Ministry shall promote wellness through education and enlightenment with balance of mind, body and spirit.

  1. AFHA is a creation of free men and women to associate with each other in the private domain according to their ministry and talents.

  2. AFHA is a platform on which members may, for AFHA’s good purpose, conduct all manner of private speech and business with AFHA, members, and other associations – keeping all business in the private domain and utilizing the protections thereof.

  3. AFHA is a private, unincorporated ministry that operates as much as possible, outside the jurisdiction of government entities, agencies, officers, agents, contractors, and other representatives, as protected by law.

  4. AFHA lawfully stands on the authority our Creator gives to free men and women as revealed in the Holy Scriptures, the Constitution of the United States of America, the constitutions of the several states of the union, and the Canadian Charter of Rights and Freedoms, as well as basic universal human rights.

  5. Members claim the right to freedom of religion, free speech, petition, assembly, and the right to gather in association to assert our rights protected by those Constitutions, Charter, and Universal Human rights.

  6. Members claim the right to be free from unreasonable search and seizure, the right to not incriminate ourselves, and the right to freely exercise all other unalienable rights as granted by our Creator, our almighty God, and guaranteed by those Constitutions, Charter, and Universal rights.

  7. Members decide for themselves which member(s) qualify to provide them counsel and advice concerning all matters, including but not limited to, physical, spiritual, healthcare, law, and any other matter, and may contract for counsel, advice, services, etc. they believe assist their ministry through AFHA.

  8. Members have the freedom to choose and perform for ourselves the types of therapies and treatments we think best for diagnosing, treating, and preventing illness and disease and for achieving and maintaining optimum wellness, as well as the freedom to choose any types of assistance in lawful matters and any other private business activity.

  9. AFHA will recognize as a member any natural person(s), (or their association) irrespective of race, color, or religion. who has joined AFHA or its social media groups and has agreed to the terms of membership, providing said person has not been sanctioned, exercised, or otherwise banned by AFHA.

  10. AFHA’s trustees, or their designee, may, at any time, terminate my membership should they conclude I am interacting with them or other members in a way that is contrary or detrimental to the focus principles, and betterment of AFHA.

  11. Member agrees that complaints, grievances, or disputes against AFHA, members, trustees, or other staff persons will be settled by a designee, committee, or tribunal chosen from Members of AFHA or Members of the Divine Purpose Ministry Association/Affiliation of Churches of which AFHA is a member. Member specifically and voluntarily waives any right to resolving any such disputes in the Courts of man (State, local and federal courts), and waives all rights to pursue a lawsuit against AFHA, its Trustees, staff or other Members. This Agreement extends to Members of other faith-based PMA organizations which are members of the Association/Affiliation of Churches under Divine Purpose Ministry. Further, Member agrees this waiver is voluntary and for the benefit of AFHA and its members, and consistent with Biblical authority (I Corinthians 6:1-5). As consideration of this agreement, Member recognizes the high costs of insurance which drives the cost of care out of range of affordability. In exchange for affordability in help with wellness issues, Member voluntarily waives on his/her behalf as well as for their family and heirs, all rights to bring any disputes to a court of man and government, preferring to rely upon Biblical principles to resolve disputes.

  12. Member understands member activity is under common law rather than statutory law or regulatory law, which are creations of public government for the public.

  13. In my relations as member, I voluntarily change my capacity from that of a public person to that of a private member.

  14. My activities within AFHA are matters of private contracts that I refuse to share with Local, State, or Federal investigative or enforcement agencies. I agree not to pursue legal action against a fellow member of AFHA unless that member has exposed me to a clear and present danger of substantive evil that would shock the conscience of any reasonable person of faith, and upon the recommendation and approval of Trustees of AFHA or Trustees/advisors from the Association of Churches, Divine Purpose Ministry. In that case only, this paragraph supersedes Para. 13.

  15. I do not, and will not while a member, represent or report to any Local, State or Federal agency whose purpose is to regulate and approve products or services or to carry out any mission of enforcement, entrapment or investigation.

  16. I may, by written notice to AFHA, withdraw from this agreement and terminate membership at any time. I will not misrepresent myself as being a member beyond the term of my membership. Upon termination of membership, my duty to confidentiality remains.

  17. AFHA may revoke my membership if I engage in abusive, violent, menacing, destructive or harassing behavior towards any other member of AFHA.

  18. These pages consist of the entire agreement for membership in AFHA.

  19. I enter into this agreement freely, without duress or coercion.

  20. I hereby exercise my right of “freedom of association” as guaranteed by the Universal Declaration of Human Rights (UDHR), the U.S. Constitution, and equivalent provisions of the various State Constitutions, as well as the Charter of Rights of Canada.

  21. I agree this contract began on the date of my joining AFHA whether online or in person. I declare that by joining AFHA and/or AFHA’s websites and/or social media group(s), I have carefully read this document and I understand and agree with it.
